I pledge on my honor that I shall not seek, give, or receive information that would compromise the integrity of the competition. I will be a kind and collaborative citizen of the OMEGA/AMM community and strive to champion the fair treatment of others.

I am voluntarily entering into this release, as part of my child’s participation in activities taking place in the American Masters of Mathematics ("Activity”) hosted by the National Mathematical Engagement Foundation, dba OMEGA Math (“OMEGA”) and I agree to the terms set forth below.

I understand that my child’s participation in the Activity is voluntary. I understand that participation in the Activity may involve physical and recreational activities which require physical exertion and may involve substantial risk of bodily injury, death and other dangers associated with participation in physical and recreational activities. I am fully aware of the risks involved and expressly and voluntarily assume such risks.

I understand that my child should participate only in those physical and recreational activities for which they have the prerequisite skills, qualifications, and training, and that are appropriate based upon my child’s health. I understand that I should consult a physician if I have any questions about the appropriateness of my child’s participation in the Activity.

The purpose and effect of this ASSUMPTION OF RISK AND WAIVER OF LIABILITY is to release and insulate OMEGA and the Board of Directors their officers, agents, employees, volunteers, representatives, and assigns from and against any claims, suits, or causes of action for injuries, death, damage, loss or expense incurred by me or my child or caused during my child’s participation in the Activity, including, but not limited to, those caused or resulting from my child’s negligence or the negligence of any other person, group, or entity, whether intentional or unintentional.

I hereby agree that for the sole consideration of my child’s participation in the Activity, I knowingly, voluntarily, and expressly waive any and all claims and release and forever discharge OMEGA and its Board of Directors and their officers, agents, employees, volunteers, representatives and assigns from any and all actions, omissions to act, claims, damages, judgments, demands, rights and causes of action of whatever kind in nature, arising out of all known and unknown, foreseen and unforeseen bodily and personal injuries or damage to property arising out of or related to my child’s voluntarily participation in the Activity, including, without limitation, injuries or damages resulting from lack of improper supervision or breach of any statutory or regulatory duty or obligation. I hereby agree that this ASSUMPTION OF RISK AND WAIVER OF LIABILITY shall be binding upon me, my child, and our heirs, executors, administrators and assignees at law.

I understand and agree that in the event I or any other party contemplated by this ASSUMPTION OF RISK AND WAIVER OF LIABILITY initiates or attempts to initiate a claim or cause of action of any kind whatsoever, I will indemnify and hold harmless OMEGA and its Board of Directors and their officers, agents, employees, volunteers, representatives, and assigns from any liability resulting therefrom, including damages, costs, and related fees.

I HAVE CAREFULLY READ THIS ASSUMPTION OF RISK AND WAIVER OF LIABILITY AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS ASSUMPTION OF RISK AND WAIVER OF LIABILITY IS A RELEASE OF LIABILITY (INCLUDING BUT NOT LIMITED TO LIABILITY FOR NEGLIGENCE) AND I VOLUNTARILY AGREE TO THE TERMS AND CONDITIONS STATED ABOVE.

I understand that in the event my child or any other party contemplated by this Release of Liability initiates or attempts to initiate a claim or cause of action of any kind whatsoever, that I will indemnify and hold harmless the OMEGA Math and its Board of Directors and their officers, agents, employees, representatives, and assigns from any liability resulting therefrom, including damages, costs, and related fees.

THIS SECTION SHOULD BE SIGNED BY THE PARENT.

I hereby grant the National Mathematical Engagement Foundation, dba OMEGA Math, its Board of Directors, and their officers, employees, agents, representatives, successors, licensees, and assigns (collectively, “OMEGA”) permission to:

(a) Record, film, photograph, or otherwise capture my image, likeness, voice, words, verbal expressions, or other depiction, and/or that of my minor children, in a photographic, video, audio, digital, electronic, or any other medium in connection with OMEGA and the American Masters of Mathematics;

(b) Use, reproduce, modify, create derivative works, display, publish, perform, transmit, and distribute the Recordings, in whole or in part, in any medium now known or hereafter developed for any purpose, including without limitation educational, promotional, advertising, and commercial purposes (“Authorized Uses”); and

(c) Use my name and/or that of my minor children in connection with the Recordings or Authorized Uses (“Use of Name”).

I understand that neither I nor my minor children will have any right to inspect or approve the Recordings or Authorized Uses and that all such Recordings and Authorized Uses, in whatever medium, shall remain the property of OMEGA. I understand that OMEGA may use the Recordings with or without an associated name.

By signing this form, I, acting on behalf of myself and my minor children, hereby fully and forever waive and release OMEGA from any claim or liability relating to the Recordings, Authorized Uses, and Use of Name, including without limitation, invasion of privacy, defamation, false light, infringement of proprietary or moral rights, and misappropriation of name, likeness, or image.

I acknowledge and represent that (i) I am over the age of 18; (ii) I have read and understand this agreement; and (iii) I freely and knowingly give my consent to OMEGA on behalf of myself and my minor children as described herein in exchange for valuable consideration, the adequacy of which is acknowledged.
